Athletic Clearance

Athletes MAY NOT participate in any of our sports programs, including Summer sports camps, until they have received approval through the Athletic Director’s Office.  Athletic Clearance is a two-step process designed to ensure that all athletes are healthy for participation and that both parents and athletes understand the inherent risks that accompany athletic competition. Once an athlete receives Athletic Clearance, it is valid for ONE YEAR. Athletes must repeat the process each year they plan on participating in our programs.
 
The following steps must be completed in order, or your athletic clearance will be delayed:
  1. Registration on http://athleticclearance.com/

  2. A sports physical examination from a medical doctor * submitted to Athletic Director (See Form Below)

It will be helpful to have the following information handy to allow for accurate completion of your online registration:
  1. Your insurance information, including the policy number
  2. Your physician information
  3. Emergency contact names and numbers

Physical Examination

Complete a sports physical examination with a doctor (M.D.). The Hart District and CIF will no longer accept physical examinations conducted by chiropractors. Please use the provided school district forms and fill in the Medical History page prior to your examination. Once completed, these forms are the only papers you need to submit to the Athletic Director.
 
Completed physical forms can be dropped off with the receptionist in the front office, whereupon they will be delivered to the Athletic Director's office.
 

We understand that physical appointments through some doctor’s offices are not easy to come by. Twice a year, the Saugus High School Athletic Training Program offers physical exams from a licensed M.D. for the price of $25. All proceeds go to benefit the Athletic Training Program.
